Phototherapeutic Keratectomy (PTK)
When cornea scarring or opacity involves the anterior layers of the cornea below the surface epithelium, and a debridement or superficial keratectomy cannot reach the deeper scarring or haze, a phototherapeutic keratectomy (PTK) procedure may help by using an excimer laser to clear the cornea.
Anterior corneal scarring or haze can cause blurry vision or irregular astigmatism. These symptoms can affect activities of daily living and quality of life.
The PTK procedure can be performed in an outpatient laser center. The laser machine used for PTK is similar to that used for LASIK. The treatment differs, as the goal is to clear the opacity, and not vision correction without glasses. After the eye surface is numb, the epithelial cells are smoothed away, exposing the underlying scar tissue. The laser is used to buff away the opacification. A bandage contact lens is the placed on the eye to protect the healing cells and minimize post-operative discomfort. Eye drops are applied regularly and healing time is generally 6-8 weeks. The laser induces vision changes, most commonly causing an eye to become more far-sighted. For most patients, this means that a change in glasses or contacts will be necessary for best vision.
